Just got this on ng+, only difference between ng and ng+ is in ng gave the apple to the lady, this is time didnt even talk to her

you probably have to give the girl the apple before beating romeo to get the outfit. I know the flavor text says she wore it on stage when she performed as the prince, so that would make sense.

There needs to be some extra step in getting this outfit, after killing Romeo it didn't drop for me

didn´t even find the NPC in the Opera house and this still dropped.
